Disciplinary Reports
The Football Association has designed Misconduct and Caution Forms that can be sent electronically to the County Football Association.
Two forms are available for a referee to download and report cautions, sending offs and misconduct.
For games played within County Durham completed forms should be Emailed to Durham County Football Association at the Email address opposite.
Advice for referees on how to complete these forms can be found on the link to Football Association’s website.
